At Excel Therapy & Conditioning we strongly believe in improving our patient's way of life and athletic/functional level to even better than it was before injury or surgery. In an effort to ensure this happens you will only see one therapist throughout your rehabilitation in order to make every effort to maintain only the highest level of consistency and care for you.
A patient evaluation is required prior to any treatment for each injury or complaint
Length and Content of PT Session is based upon therapist recommendation following PT Evaluation
Treatment plan will be discussed with patient prior to any delivery of service or charge
Check to see if your insurance is accepted, or if your insurance company will reimburse charges- otherwise treatment is fee for service.
Out of Pocket Fees will be discounted when paid in CASH or DEBIT and AT TIME OF SERVICE
